A randomized clinical trial published in <em>Brazilian Journal of Medical and Biological Research<\/em> compared the circular stapler with the conventional surgical technique in a group of 879 adult men. The results clearly demonstrate the superiority of the stapler method in key parameters.  <\/p>\n\n<h2 class=\"wp-block-heading\">Methodology: Randomization, 4 Centers, 879 Patients<\/h2>\n\n<p class=\"wp-block-paragraph\">The study included 879 adult men (18\u201370 years) with excess foreskin or phimosis, treated in four urology departments between 2013 and 2014. Patients were randomly divided into two groups: <\/p>\n\n<ul class=\"wp-block-list\"><li>441 patients \u2014 <strong>circular stapler<\/strong> circumcision,<\/li><li>438 patients \u2014 <strong>conventional<\/strong> circumcision using the dorsal slit technique (according to WHO guidelines).<\/li><\/ul>\n\n<p class=\"wp-block-paragraph\">The study received bioethics committee approval, and all patients provided informed consent. Severe edema occurred in 5 out of 441 patients (stapler) compared to 18 out of 438 (conventional method). In the stapler group, <strong>no wound dehiscence was observed<\/strong>, while in the conventional group it occurred in 5 patients. The authors attribute these benefits to the short procedure time, minimal tissue trauma, and absence of electrocoagulation. In the entire group of 879 patients, no penile deformity or long-term complications were observed.    <\/p>\n\n<h2 class=\"wp-block-heading\">Method Limitations \u2014 Honestly<\/h2>\n\n<p class=\"wp-block-paragraph\">A reliable assessment requires identifying weaknesses:<\/p>\n\n<ul class=\"wp-block-list\"><li><strong>Cost<\/strong> \u2014 the stapler procedure was significantly more expensive.<\/li><li><strong>Staple removal<\/strong> \u2014 in 88% of patients (388\/441), removal of residual staples by a surgeon was necessary, as they did not always fall off spontaneously. Additionally, 9.5% of patients required intraoperative suturing. <\/li><\/ul>\n\n<p class=\"wp-block-paragraph\">The staple issue was identified by the authors as the main direction for further improvement of devices in this class.<\/p>\n\n<h2 class=\"wp-block-heading\">Anastomat ZSR 2.0 in light of this data<\/h2>\n\n<p class=\"wp-block-paragraph\">The Anastomat ZSR 2.0 belongs to the class of circular staplers whose efficacy and safety are confirmed by the above study. The main challenges \u2014 cost and staple removal \u2014 are being addressed by subsequent generations of devices.
